1) Not Medical Advice

The Site provides general information only. It is not medical advice and does not replace evaluation by a clinician. Do not rely on the Site for diagnosis or treatment decisions.

2) No Emergencies

Do not use the Site for emergencies. If you believe you have a medical emergency, call 911 or go to the nearest emergency room.

3) No doctor–patient relationship created by the Site

Using the Site, sending a message, or requesting an appointment does not create a doctor–patient relationship. A doctor–patient relationship is established only after you complete the Practice’s intake process and the Practice confirms acceptance of you as a patient.

4) Contact forms, appointment requests, and response times

If the Site allows you to contact the Practice:

Messages may not be reviewed immediately.
Appointment requests are not confirmed until you receive confirmation from the Practice.
Do not include sensitive medical information in a general web form or unencrypted email. Use the patient portal or call the office.

5) Patient portal and third-party services

The Site links to an external patient portal (Elation Passport) and may link to other third-party sites (including Dr. Kordonowy’s Substack). Third-party services are not operated by the Practice, and their terms and privacy policies apply when you use them.

6) Acceptable use

You agree not to:

Use the Site for unlawful, harmful, or fraudulent purposes
Attempt to access non-public areas or interfere with Site security
Introduce malware or attempt to exploit vulnerabilities
Scrape or harvest information from the Site without permission

We may suspend or block access for misuse.

7) Intellectual property

The Site and its content (text, images, logos, design) are owned by or licensed to the Practice and are protected by intellectual property laws. You may use the Site for personal, non-commercial purposes only.

8) Disclaimers

The Site is provided “as is” and “as available.” We do not guarantee the Site will be uninterrupted, error-free, or secure.

9) Limitation of liability

To the fullest extent permitted by law, the Practice is not liable for indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Site.

10) Changes to the Site or Terms

We may update the Site and these Terms at any time. Updated Terms will be posted with a new “Last Updated” date. Continued use of the Site means you accept the updated Terms.

11) Governing law and venue

These Terms are governed by Florida law, without regard to conflict-of-law principles. Venue for disputes relating to the Site will be in state or federal courts located in Lee County, Florida, unless otherwise required by law.
