Your first visit is comprehensive. We review your full medical history, lifestyle, and goals.
Note: All initial visits are IN-PERSON ONLY at our Fort Myers office.
To secure your 60-minute comprehensive review, a non-refundable professional fee of $150 is required.
Do you take insurance?
We operate independently of insurance networks to ensure your care is dictated by medicine, not billing codes. However, you may use your insurance for all external needs, including labs, imaging, and prescriptions.
Medicare?
We have opted out of Medicare to prioritize the physician-patient relationship. While our professional services are private-pay, your Medicare coverage remains fully valid for all hospitalizations, specialist referrals, and external testing at any Medicare-accepting provider or location.
By submitting this form, you acknowledge that no patient-physician relationship is established until an initial visit occurs.
