When you receive your standard lab results, you usually see ‘Total Cholesterol,’ ‘HDL,’ and ‘LDL.’ While these provide a baseline, they are often insufficient for identifying true cardiovascular risk. In fact, many individuals who suffer heart attacks have ‘normal’ LDL levels according to standard guidelines.

Think of LDL-C (concentration) as the weight of the cargo in a truck, while LDL-P (particle number) is the number of trucks on the road. It is the number of particles that ultimately determines how likely they are to penetrate the arterial wall and cause plaque buildup.

The "Normal" Cholesterol Paradox

LDL-C measures concentration, not the number of particles. Two patients can have the same LDL-C value but vastly different particle counts.

Why does this matter? Because small, dense LDL particles are more likely to penetrate the arterial lining and cause atherosclerosis. A patient with predominantly large, buoyant LDL particles may have a higher LDL-C but lower actual risk. Conversely, a patient with small, dense particles may have ‘normal’ cholesterol but be at significant risk.
